How can I get a seaman's book here in the Philippines?

Where to apply:

You must secure all the requirements needed, then file them at the Maritime Industry Authority (MARINA), located at the PPL Bldg., 1000 U.N. Ave. cor San Marcelino, Manila.

REQUIREMENTS:
Transcript of Record (High School TOR for High School Graduates) (College TOR for College Graduates)
School Diploma (High School Diploma for High School Graduates ) (College Diploma for College Graduates)
Certificate of Authentication and Verification (CAV)
Certified True Copy of Birth Certificate
NBI Clearance - visa seaman
2 pcs Passport size colored picture with white background
Basic Safety Course Certificate

    update:
    Birth cert
    passport
    Basic Safety Training Certificate
    OR of BST Cert
    php 815 for 24 hrs
    php 1515 for 4 hrs processing
    and php 20 fee for uniform for picture taking..

    me idea po ba kayo if ever nawala ung OR of BST cert kung anong dapat gawin/ alternative action? kac po nawala ung OR ng kapatid ko and neither the school or the PNTI issued another OR or kahit Certficate man lang na nagpapatunay na nakabayad sya.     Seaman Book is an identity book which is issued to the candidates who have stcw certificates. It is used when you board the ship, that is why it is known as seaman identity book.

    hi pwedeng mag ask po sa inyo..gusto ko po kasing mag seaman ano po bang kailangan na training para po maka pag seamans graduate ako ng electrical tech..at kasalukuyan po akong ngwowork ngayon. .work ko po dito E&I technician. .gusto ko po sana pag uwe ko maka pagtraining ako. .

    Hello po, college graduate computer engineering ang husband ko. Pero we decided n sumubok magwork as seaman since walang suerte sa landbase posible po ba or malaki ba ang chance anong mga requirements ang necessary.

    I'm a hold a degree in Computer Science already and has just finished my marine Engineering course. I already have my TOR for my first course and would still need to wait for the TOR of MarE. Can I use my old TOR to apply for the document? Or should I wait for my MarE TOR? THanks
